I have not heard the term "fully rotated astigmatism" before. Astigmatism can be regular and irregular.
A number of things cause astigmatism including hereditary factors, trauma, previous surgery (especially if sutures are left in the cornea), and corneal disorders. Various corneal diseases can cause astigmatism, such as keratoconus (a cornea that becomes thin and misshaped over time)or corneal scars.
