Answer:
Any abnormality discovered during a vision screening exam should prompt one to refer the patient to an ophthalmologist. Leaning forward suggests they may need glasses. Turning the head to one side could suggest a few things, but may indicate an abnormality with one of the extraocular muscles or a retina problem within the eye.
