Question:
Any idea why a 59-year-old male would have recurrent subconjunctival hemorrhage in the same spot in the right eye after cataract surgery? Vision is great, but every two weeks, it happens again. My medical workup is negative and I'm on no ASA or Coumadin-like drugs. It's been going on for two years like clockwork.
Answer:
Similar cases have been reported, and there are a multitude of possibilities. In general these can be lumped into: 1) "bleeding disorders" (seems to have been ruled out in this case); 2) localized vascular anomalies (a board-certified ophthalmologist should be able to diagnose this; and 3) "associated problems," including localized trauma such as touching eye when asleep, dry eyes with excessive touching, etc. A thorough eye examination will increase the odds of discovering a cause, although this is not always possible. Fortunately, the hemorrhages cause no damage in spite of appearing "bad."
