Yes, you must remove all eye makeup prior to surgery to prevent contamination of the surgery by infection-causing germs. Your ophthalmologist will likely also counsel you against using any eye makeup after the cataract surgery for the same reason and to protect against you damaging the surgery when removing your eye makeup at night or worse, leaving it on.
I know we all want to look as good as we can, but this is one instance when seeing good is more important than looking good—at least for a short time.
