Answer:
To the best of my knowledge there is no law that requires that a patient be given the identification card for their surgical implant, eye or other implant. There is a law that the manufacturer must track the information to the facility where the implantation takes place. The surgical facility is also required to track the same data. It is the custom for most doctors and facilities to give their patients the patient identification card that comes with the implant, but it is not a law to do so. It is possible that a patient would refuse to give their correct name or proper social security number.
